How does it work? Migraine is a neurovascular headache. Sumatriptan works by narrowing blood vessels in the brain. Sumatriptan succinate is a vascular serotonin receptor agonist. It stimulates 5-HT receptors of the 1D subtype. This specific receptor subtype is present in the cranial and basilar arteries and mediates vasoconstriction. Activation of these receptors causes vasoconstriction of dilated arteries. Sumatriptan also blocks the release of vasoactive neuropeptides during migraine and may inhibit the release of inflammatory mediators from the trigeminal nerve. Imitrex is an excellent migraine-abortive agent, but not a preventive. Dosage It is advisable to take Imitrex as soon as you notice symptoms of a migraine attack. The initial dose for adults is 25, 50, or 100 mg taken with water or other liquid. The most you should take at one time is 100 mg, and the most you should take in 1 day is 200 mg. If you have liver disease, you should not take more than 50 mg at one time. If your migraine goes away and then comes back, or if you have a partial response to the first dose, you may take a second after at least 2 hours. If the first dose has not helped you even slightly, then Imitrex probably will not work at all. In the United Kingdom, sumatriptan (Imigran Recovery) has been available without a prescription since 2006.
