Mechanism of action

Acyclovir disrupts the process by which the virus makes copies of itself and spreads to other cells.

To be active, acyclovir must undergo phosphorylation. It is phosporylated to its monophosphate by a virus-encoded thymidine kinase. Acyclovir is activated only in the infected cells, and is active only against actively replicating viruses. It cannot eradicate the latent virus.

How to take

  • Acyclovir may be taken with or without food.
  • Antiviral nucleoside analogues do not cure the herpes virus or prevent its transmission.
  • Ask your health care provider for advice if acyclovir (Zovirax) does not seem to be controlling lesions or frequency of outbreaks.
  • Changing the dosage regimen without doctor's approval may result in decreased effectiveness and herpes outbreak.
  • Do not add acyclovir to biologic or colloidal fluids. Precipitation may occur with bacteriostatic water.

Acyclovir dosage

Genital herpes:
Adults:
Initial episode: 200 mg every 4 hours while awake (5 times/day) for 10 days; 400 mg 3 times/day for 5-10 days has also been reported.
Recurrence: 200 mg every 4 hours while awake (5 times/day) for 5 days; 400 mg 3 times/day for 5 days has also been reported.
Chronic suppression: 400 mg twice daily or 200 mg 3-5 times/day, for up to 12 months followed by re-evaluation.

Herpes zoster (shingles):
Adults (immunocompetent): 800 mg every 4 hours (5 times/day) for 7-10 days.

Varicella-zoster (chickenpox):
Children >= 2 years and <= 40 kg (immunocompetent): 20 mg/kg/dose (up to 800 mg/dose) 4 times/day for 5 days.

Adults and Children >40 kg (immunocompetent): 800 mg/dose 4 times a day for 5 days.
