Detailed Information

Acyclovir is an acyclic analogue of the natural nucleoside, guanosine. It is a prescription antiviral drug used to treat herpes infections of the skin, mouth, genital herpes, herpes zoster (shingles), and chickenpox. It slows the growth and spread of the HSV. This drug does not cure herpes, but relieves the pain and may make the infection clear faster.

Acyclovir is highly active against herpes simplex virus (types 1 and 2) and varicella zoster virus. It has activity against several other herpes type viruses, e.g., Epstein-Barr virus, pseudorabies, but only slight activity against the human cytomegalovirus.

Indications:

  • treatment of initial episodes and the management of recurrent episodes of genital herpes
  • chronic suppressive therapy for recurrent disease
  • acute treatment of herpes zoster (shingles)
  • treatment of chickenpox (varicella)

Acyclovir Off-label uses:

Acyclovir vs. Valacyclovir (Valtrex)

Valacyclovir has several advantages over acyclovir: exhibits significantly greater oral bioavailability, results in higher blood levels, and can be given less frequently. It is at least as efficacious. However, valacyclovir is more expensive than off-patent acyclovir.

In herpes zoster, valacyclovir is more effective than acyclovir or placebo (and as equally effective as famciclovir) in shortening the length and severity of herpes zoster-associated pain and postherpetic neuralgia.

In the management of herpes zoster, valacyclovir significantly accelerate the resolution of herpes zoster-associated pain compared with acyclovir.
